Renovation of a mixed-use development in Bedford, Massachusetts. Completed plans call for the renovation of a medical facility.
Contractor shall provide and install two Exacq rack mount NVRs (Model IP08-12T-2AW-E) at the Edith Nourse Rogers Memorial Veterans Hospital in Bedford, MA, one in Building 4 basement and one on Building 78, 3rd floor mental health, and shall program a total of thirteen existing cameras into the new NVRs (three cameras in Building 4 and ten cameras in Building 78). Contractor shall provide all labor, equipment, material, accessories, and additional camera licenses as needed to perform the work and fully integrate the new NVRs with the existing camera system. The work shall be performed in a fully operational medical center environment and must be planned, coordinated, and scheduled to maintain continuous services to all occupied areas, minimize disruption to patients and staff, and include off-hours work when necessary. Contractor shall thoroughly review the site and existing conditions, verify all dimensions and utilities, and assess impacts on adjacent occupied spaces prior to starting work, notifying the VA of any discrepancies, construction complexities, or required changes before cost agreements are finalized. All work, materials, and workmanship shall conform to applicable local and national standards, VA standard operating procedures, industry recommended practices, safety codes, and the Massachusetts state building code, with all required notifications, approvals, and permits (including ILSM and ICRA) obtained prior to starting work. Contractor is responsible for all construction means, methods, sequencing, scheduling, procedures, and safety precautions, and shall provide adequate numbers of skilled workers familiar with the required methods. Contractor shall protect all existing buildings, improvements, utilities, and equipment, including piping, conduits, wires, cables, fire protection, communications, and other systems that are not scheduled for removal or abandonment, immediately repairing at its own expense any damage caused by its operations. Cutting, altering, or removing structural work or disturbing ducts, plumbing, steam, gas, or electrical work requires prior approval from the Contracting Officers Representative (COR). Existing work that is altered or extended and found defective shall be reported before it is disturbed. Surfaces and systems disturbed or removed to perform new work shall be patched, repaired, reinstalled, or replaced and refinished to at least the condition that existed prior to construction. Contractor shall keep the site clean, dispose of all debris legally as part of the work, maintain clear and safe access, and broom-clean the work area daily. All storage locations, use of common areas, dumpsters, and equipment staging must be coordinated with and approved by the COR. Contractors employees and subcontractors shall comply with all VA security management programs and VA Police direction, check in with government-issued photo identification to obtain badges, remain only in areas required for their work, and be subject to inspection when entering or leaving the site. No photography of VA premises, residents, patients, or staff is allowed without permission of the Contracting Officer or COR. Contractor shall submit a safety plan and comply with Interim Life Safety Measures and Infection Control Risk Assessment requirements, including completing and posting required forms and implementing maximum feasible protection of surfaces and occupied areas, with protection installed and removed as needed and as directed by the COR. Any incidents, including unplanned utility disruptions, flooding, or on-site accidents, shall be reported to the COR as soon as possible. All contractor vehicles shall be parked only in designated parking areas or construction yards, and work activities shall be conducted so as to allow continued and safe 24/7 operations of the medical center. Contractor shall maintain a record set of drawings, specifications, sketches, and related documents on site, clearly labeled and updated to reflect as-built conditions and any changes. Products shall be provided as specified, with named manufacturers where indicated, and pricing shall be based on specified items; substitutions may be proposed only as deduct alternates offering cost savings and must receive written approval before use. Contractor shall provide a construction schedule within five business days of project approval, showing start and completion dates, dependencies, approval dates, mobilization, individual trade activities, inspections, substantial completion, move-in, and close-out, and shall keep this schedule updated throughout construction. The contractor shall coordinate all work and schedule changes with the COR in Engineering, who will facilitate coordination with end users; the COR must be copied on all correspondence. Requests for Information (RFIs) must be submitted in writing, and any change orders must be approved in writing before associated work is performed. Contractor and subcontractors shall warrant all work for a period of one year from the date of final acceptance. The contractor shall review and stamp all submittals (including subcontractor submittals) confirming verification of field dimensions, adjacent construction, environmental and job conditions, coordination with other work, and conformance with contract documents before submitting them to the VA for review, and shall provide samples of all finish materials. Near completion, the contractor and COR shall conduct a walkthrough to identify deficient or incomplete items, for which the contractor shall provide a completion schedule. At final completion, the contractor and COR shall review the work and jointly sign off on completed items. Contractor shall comply with all applicable Federal records management laws and regulations and National Archives and Records Administration (NARA) policies, including the Federal Records Act, 44 U.S.C. chapters 21, 29, 31, and 33; 36 CFR Chapter XII, Subchapter B; the Privacy Act of 1974, 5 U.S.C. 552a; and FOIA, 5 U.S.C. 552, as amended. All data and records created for Government use, delivered to, or under the legal control of the Government in the performance of this contract are Federal records and must be preserved, managed, and scheduled for disposition only as permitted by statute and regulation. Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use. The contractor and Bedford VA Health Care System are responsible for preventing alienation or unauthorized destruction of records; records may not be removed from VA custody or destroyed except in accordance with approved records schedules and with written concurrence of the Head of the Contracting Activity. Any unlawful or accidental removal, defacing, alteration, or destruction of records must be reported to the agency, which will report promptly to NARA under 36 CFR 1230. The contractor shall immediately notify the Contracting Officer upon discovery of any inadvertent or unauthorized disclosure of non-public information, and shall establish appropriate administrative, technical, and physical safeguards to protect the security and confidentiality of all information, data, documentary material, records, and equipment. Contractor shall not remove such materials from Government facilities or systems without express written permission, and when information or records are no longer required, they must be returned to Government control or held until otherwise directed. Destruction of records is expressly prohibited except as allowed under approved schedules. The contractor must obtain Contracting Officer approval prior to entering any subcontract that requires disclosure of information, documentary material, or records generated under or related to this contract, and shall ensure all subcontractors comply with all Government and Bedford VA Health Care System guidance on protecting sensitive, proprietary, classified, and controlled unclassified information. Contractor shall only use Government IT equipment for purposes specifically tied to or authorized by the contract and shall not create or maintain records containing non-public information except as specifically tied to or authorized by the contract. The contractor shall not retain, use, sell, or disseminate copies of any deliverable containing Privacy Act protected or otherwise non-public information, and Bedford VA Health Care System shall own the rights to all data and records produced, with unlimited rights to use, dispose of, or disclose such data as deemed in the public interest, subject to any identified contractor rights under FAR 52.227-11 through 52.227-20. All contractor employees assigned to this contract who create, work with, or handle records must complete VA-provided records management training, including any required initial, annual, or refresher training, and the contractor shall ensure and document completion. All of these records management and safeguarding requirements shall be flowed down to all subcontractors, with written acknowledgment, and any subcontractor violations will be attributed to the prime contractor. The contract duration for completion of all required work is 60 days from award. Contractor shall complete the following: 1. Bldg 4 - Provide and insall (1) Exacq Rack Mount NVR Model IP08-12T2AW-E - Program (3) existing cameras into the NVR 2.
