Sealed proposals are requested for the replacement and/or repair of concrete sidewalks, curbs/ramps, and drive approaches at various locations in the City of Cuyahoga Falls. Bidders must use the printed forms provided; each proposal must include the full name of all parties involved and a bond or certified check for 5% of the total bid amount as a guarantee. Only those who have obtained a recorded bid set of drawings and specifications may submit bids. The successful bidder must provide a Performance, Payment, Maintenance bond in the amount of 100% of the total bid. The use of domestic steel is required according to state law. The City of Cuyahoga Falls applies a Local Bid Preference. All parties are encouraged to attend a pre-bid meeting. The Director of Public Service reserves the right to reject any or all bids and waive informality in proposals. The City is an Equal Opportunity Employer. Further details, specifications, and forms are available from the City Engineer's office. The work shall be performed at various locations within the City of Cuyahoga Falls. Locations are distributed randomly throughout the City. SCOPE OF WORK The Contractor shall furnish tools, equipment, materials, labor and services for the complete removal and construction of sidewalk, various curbs, ramps and all construction necessary and associated therewith. The work shall include but not be limited to the following: 1) Removal of sidewalk, various curb, drive approach and appurtenances at locations specified by the City Project Inspector. 2) The forming, pouring and finishing of sidewalk, various curbs drive approach & ramps at locations that had been removed. 3) Restoration of lawn areas, trees, fences, streets or any appurtenance located in adjacent areas damaged by performance of the above work. 4) Excavation, installation and backfilling of water valve boxes located in removed sidewalk as directed by the City Project Inspector. 5) Patching of existing concrete driveways with new concrete to meet replaced concrete. 6) Asphalt appurtenances abutting against sidewalk shall be patched with ODOT Specification 448 Hot Mix Asphalt to make a smooth transition between asphalt and concrete. 7) Replacement and installation of house drains. This will include securely hooking the new drain up to existing drains with a watertight connection. 8) Construction of New Sidewalk, Per ODOT Specification 608 Walks, Curb Ramps and Steps 9) Prepare base, Per ODOT Specification 203 Roadway Excavation and Embankment for drive approach, new sidewalks, and various curbs.
