Renovation of a mixed-use development in Fort Wayne, Indiana. Completed plans call for the renovation of a medical facility.
Replace and relocate the backflow preventer in Building 1 includes providing all labor, equipment, materials, and supervision necessary to perform the required plumbing work in accordance with the salient characteristics attached to the notice. The effort supports market research to determine whether sufficient certified Service-Disabled Veteran-Owned Small Businesses or Veteran-Owned Small Businesses are available and capable of performing the work to justify a set-aside and ensure fair and reasonable pricing in accordance with the VA Rule of Two. Firms must be certified in the SBA Veteran Small Business Certification database and able to perform the work under NAICS 238220, Plumbing, Heating, and Air-Conditioning Contractors. Location: Fort Wayne VA Medical Center, 2121 Lake Avenue, Fort Wayne, IN 46805. Specifics: Contractor will provide all labor, equipment, materials, and supervision necessary to replace and relocate the backflow preventer in Building 1. 1. Remove 4-inch backflow preventer from ceiling and disposal of existing device and associated materials. 2. There are two isolation valves attached to this preventer that will also be replaced. 3. VA will provide the following materials for items 1 and 2 above: (a) New backflow preventer. (b) Two isolation valves. 4. Run piping from existing water main in ceiling and pipe to VA supplied backflow preventer at ground level. 5. Prefab piping to be complete before Water shut down to occur to minimize time domestic water is turned off. 6. Make stand for backflow preventer support. 7. Contractor shall provide all labor and materials to replace one 6" gate isolation valve. This is a shut valve on a double check valve. 8. Test and submit all proper paperwork to COR. Testing shall be done by a certified backflow tester. 9. Prep-work shall be done before outage to minimize time domestic water is turned off. 10. All prep-work can be done during normal working hours, 7:00am - 4:00pm, Monday through Friday. 11. The actual outage and tie-in of the backflow preventer shall be done outside of normal working hours. Water outage request shall be submitted to COR at least two weeks prior to the outage. 2) Regulations and Requirements. 1. Contractor shall provide a site-specific safety plan. 2. Contractor shall provide a proposed performance schedule before the start of work. 3. Contractor shall provide an estimate to the COR before start of work. 4. Contractor shall provide submittals to COR prior to installation. 5. The CO will issue the Notice to Proceed before any work or mobilization is authorized to begin. 6. Contractors shall clearly include the solicitation number (prior to award) or the contract number (as appropriate) on all documents submitted to the VA. 7. "No Government personnel, other than the Contracting Officer, have the authority to change or alter these requirements. The COR shall clarify technical points or supply relevant technical information, but no requirements in this scope of work may be altered as a sole result of such verbal clarification." 3) Retainage a) The Contracting Officer may retain funds-- (i) Where performance under the contract has been determined to be deficient or the Contractor has performed in an unsatisfactory manner in the past; or (ii) As the contract nears completion, to ensure that deficiencies will be corrected, and that completion is timely. b) Examples of deficient performance justifying a retention of funds include, but are not restricted to, the following-- (i) Unsatisfactory progress as determined by the Contracting Officer. (ii) Failure to meet schedule in Schedule of Work Progress. (iii) Failure to present submittals in a timely manner; or (iv) Failure to comply in good faith with approved subcontracting plans, certifications, or contract requirements. The Method of Contractor Selection has not been determined at this Time.
