The work consists of CCTV inspection of the following: 30-inch HDPE pipe (4,124 LF, C01 to C09 and C11 to C12), 12-inch HDPE pipe (469 LF, C09 to C11), 24-inch pipe cured-in-place pipe lining (991 LF, C09 to C11), 18-inch storm drain (~200 LF, C06 and C07), 8-inch HDPE sliplined pipe (~110 LF, C05), 8-inch PVC sewer main (350 LF, C07), 23 manholes (C01 to C12), and 3 vaults (C02 and C07). Manhole access for the 24-inch CIPP segments are up to 30 feet in depth and all manholes will require appropriate confined space entry, safety, and ventilation equipment if any manned entry is intended to be performed. Contractor shall provide as deliverables CCTV video files of each segment identified above, a CCTV report that identifies structural and O&M defects, and a red-line markup indicating locations of significant defects worthy of repair. Proposals shall include: 1. Experience of firm’s CCTV experience for small and large diameter pipe. 2. Indication of requisite lighting and camera quality to identify defects in large diameter pipe. 3. Statement of PACP and/or MACP certification for the CCTV operator. 4. Schedule for the duration of the services provided, including hours allocated to each staff member and charge out rate. 5. Provisions for onsite inspection, additional CCTV inspection, and quality control/assurance as necessary. 6. Agreement to the terms of the attached maintenance services agreement, with minor modifications considered.
