Sealed bid proposals are invited for the Phase I Signage Project. Each bid must include the full name(s) of all parties submitting the bid, a fully executed Non-Collusion Affidavit, for corporations not chartered in Ohio: a certificate authorizing the corporation to do business in Ohio (Articles of Incorporation, listing principals), a certified or cashier's check or bid bond equal to 10% of the total bid amount payable to the City of Beachwood, proof of General Liability Insurance from a company licensed in Ohio with a policy amount of $1,000,000.00 listing the City of Beachwood as an additional insured, and proof of Workers' Compensation coverage. The bid check or bond guarantees entering into an agreement with the City if awarded the contract, with forfeiture of the bid amount as liquidated damages if the agreement is not executed. A Performance Bond for the full contract amount is required before contract execution. Minority Business Enterprises (MBE) and Women's Business Enterprises (WBE) should include a Certification Letter.
