The project involves street milling/reconstruction and hot mix asphalt placement, including sidewalks, curbs, and ADA Ramps as described in the specifications. Bidders are instructed to submit sealed bids using Indiana Form No. 96 with the appropriate financial statement. Each bid must include a certified check or acceptable bid bond of not less than five percent of the total bid amount. Bids must be in sealed envelopes marked with the name and place of business of the bidder. Required documentation includes E-Verify employment documentation, City Nepotism documentation, Doing Business with Iran, and Tier 1 Contractor Certification Pursuant to IC 5-16-13. Upon award of the contract, a performance bond equal to the contract price and a payment bond to insure payment of subcontractors, contract laborers, material suppliers, and service providers are required.
