Each bid must be accompanied by a bid bond, cashier's check, or certified check for ten percent (10%) of the total bid, not to exceed $20,000. A Statement of Ownership Disclosure is required, listing names and addresses of all persons and entities owning ten percent (10%) or more of any level of ownership. Pre-qualification requirements: Bidders must be prequalified by the Department of the Treasury, Division of Property Management and Construction, with a valid and active NOTICE OF CLASSIFICATION in the appropriate trade category, a certified copy of a Total Amount of Uncompleted Contracts Form, and a Prequalification Affidavit-No Material Adverse Change.
