The system shall include all equipment, accessories, and appurtenances required for a complete and operable unit. Required with bid: Signed 'Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion--Lower Tier Covered Transactions,' signed 'Certification Regarding Lobbying (EPA Form 6600-06),' completed and signed 'DBE Good Faith Effort Documentation,' and completed 'Bidders List' that includes all entities that provided the Bidder a bid or quote for this contract, whether or not Bidder proposed to utilize the entity.
