Steris Sterilization Equipment Preventive Maintenance (PM) and Repair Services are required for the Veterans Healthcare Administration Salt Lake City Health Care System (VHASLCHCS) at 500 Foothill Drive, Salt Lake City, UT 84148. The work includes maintenance and repair of Steris sterilization equipment using new OEM medical supplies, equipment, or replacement parts. Offerors must be an OEM, authorized dealer, and have authorized Steris equipment certification for services and maintenance, verified by an authorization letter or documentation from the OEM. All software licensing, warranty, and services must comply with OEM terms and conditions. Gray market items and remanufacturers are not acceptable. Offerors are required to submit a completed pricing schedule, a Steris Authorization Letter, and a VA Notice of Limitations on Subcontracting - Certificate of Compliance. If subcontractors are used, they must be listed with their size standard and role; otherwise, state that no subcontractors will be used. Quotes are to be submitted via email only.
