Clay County is requesting quotes from qualified contractors to provide housing rehabilitation services at the following location: 3647 Calvados Avenue Orange Park, FL 32065 If any additional services are requested from the homeowner, the Contractor will be required to notify the County Project Manager or designee. No additional services shall be performed until the Contractor receives written authorization from Clay County. Home rehabilitation project includes, but is not limited to the following items: A. Electrical 1. Smoke alarm installed to current code in three (3) bedrooms (communicating / linked 10- year battery minimum or new hard-wired replacements). 2. Add GFCI outlet protection as needed per code: Two (2) bathrooms and one (1) kitchen. B. Exterior Carpentry 1. Remove and dispose of existing concreate steps and replace with wooden front steps. Steps shall be per code. 2. Add wooden handrail on front of steps per building code. C. Internal Carpentry 1. Remove damaged bedroom flooring and replace sub-floor with 23/32 plywood or equivalent durable material. 2. Replace flood covering on new sub-floor in bedroom with carpet to match existing carpet in living room. 3. Remove current floor covering in dining / kitchen area if needed to access damaged subfloor. 4. Replace floor covering in dining / kitchen area to match existing floor if floor was removed for access. 5. Replace damaged sub-floor with new 23/32 plywood or an equivalent durable material. 6. Install R-19 floor insulation and vapor barrier under replaced bedroom, dining and kitchen areas. 7. Install new steps / vinyl tread with handrail for code compliance into the den. 8. Install new steps / vinyl tread with handrail for code compliance into current pantry / laundry room; only if run of steps will not require removal or reconfiguration of current appliances. Final Completion shall be obtained within sixty (60) days from Notice to Proceed. The Contractor shall be assessed Liquidated Damages (LD) of $100.00 for each day past final completion date set by the contract.
