Work on this contract consists of B-3-215: 270 CY Concrete Masonry Bridges, 465 LF Prestressed Girder Type I 36W-Inch, 710 LF Piling Steel HP 10-inch X 42 LB, Bar Steel Reinforcement HS Structures, Concrete Pavement Approach Slab HES.Bids will be received for a single prime Contract. Bids shall be on a unit price basis as indicated in the Bid Form.Bids shall be on the forms provided for that purpose and per the Contract Documents prepared by CBS Squared Inc. identified as "CTH T Bridge Over Vermillion River, LRIP/CHIS Structure Replacement". No proposal will be accepted unless accompanied by a certified check or bid bond equal to at least 5% of the amount bid, payable to the Owner as a guarantee that, if the bid is accepted, the bidder will execute and file the proper contract and bond within 15 days after the award of the contract. Bidder is required to deliver the original certified check or bid bond within 72 hours of the bid opening to CBS Squared, Inc., ATTN: Ashley Marschall, 770 Technology Way, Suite 1A, Chippewa Falls, WI 54729. The certified check or bid bond will be returned to the bidder as soon as the contract is signed, and if after 15 days the bidder shall fail to do so, the certified check or bid bond shall be forfeited to the Owner as liquidated damages.
