CURBSIDE YARD WASTE COLLECTION SERVICE and all work necessary thereof and incidental thereto, all in accordance with the Specifications, copies of which are available for inspection in the Office of the Township Clerk during regular business hours. Bidders must use, and complete fully, Proposal blanks furnished by the Township Clerk, comply with all requirements attached thereto, and accompany bids with the following: 1. Certified check or cashier's check in the amount of 10% of the Total Bid, but not more than $20,000; payable to Township of River Vale; or Bid Bond in the amount of 10% of the Total Bid but not more than $20,000 in form approved by the Township Attorney. 2. A Consent of Surety in the principal sum of one hundred (100%) percent of the contract amount 3. Bid Document Submission Checklist. 4. Acknowledgement of Receipt of Changes to Bid Document(s) 5. Copy of Business Registration Certificate 6. Non Collusion Affidavit. 7. Affirmative Action Affidavit. 8. Stockholder Disclosure Statement. 9. Acknowledgments and Statement of Organization. 10. Certification of Bidder's status on the State Treasurer's list of Debarred, Suspended and Disqualified Bidders. 11. Prohibited Russia-Belarus Activities and Iran Investment Activities. 12. Proposal properly completed in accordance with the form thereof. Proposal must be enclosed in a sealed envelope with the bidder's name and address and the designation of the work or materials noted on the outside of the envelope. The Town Council expressly reserves the right to reject any or all bids and accompanying documents received. The Town Council also expressly reserves the right to correct any erroneous mathematical computation in any proposals submitted, and to consider the bid on the basis of the corrected total or totals.
