1. All work shall be performed by individuals directly employed and supervised by the Contractor. Subcontracted work is not allowed.
2. All work performed other than specifically called for herein shall be done only after
authorization by the State and shall be at rates mutually agreed upon by both parties prior to
commencement of any work. Contract Period: 3

2. Contractor shall provide:
One (1) Three (3) yard container picked up One time per week.
Three (3) Four (4) yard containers picked up One time per week.
One (1) Four (4) yard container picked up Two times per week.
Two (2) Six (6) yard containers picked up One time per week.
Five (5) Six (6) yard containers picked up Two times per week.
Two (2) Eight (8) yard containers picked up Three times per week.
Two (2) Eight (8) yard containers picked up Two times per week.
One (1) Thirty (30) yard container picked up as needed.
Containers are to remain on-site for the duration of the contract.
3. This contract will include delivery of the container, pickups, transport and disposal of refuse.
4. Landfill charges are not included in this contract. An itemized account will need to be presented, along with a letter stating the increases. Fuel surcharges and franchise fees will not be paid.
5. Payments shall be made in monthly installments. Contractor billings shall be submitted to DFCM within five (5) business days following the end of each month Pre-Bid Site Meeting
