Description of work: Rehabilitate Runway 4-22 artial Depth Repair of Concrete Pavement, 300 S.F.; Install Expansion Joint, 400 L.F.; Pavement marking, 100 S.F. Contract Completion Time: 5 Calendar Days Have you submitted to the Engineer written or, if time is not available, oral questions about the meaning or intent of the construction documents or other inquiry, prior to the time set for the prebid conference Interpretations or clarifications considered necessary by the Engineer in response to such questions will be issued by Addenda to all parties recorded by the E-Bidding system as "plan holders" and having received the construction documents. Questions received after the prebid conference or less than seven days prior to the date for opening of Bids may not be answered. Only questions answered by formal written Addenda will be binding. Oral and other interpretations or clarifications will be without legal effect. A paper hard copy prequalification is required to be submitted PRIOR to the bid opening. The proper prequalification form must be sent to the DOT to establish prequalification and remain valid prior to the deadline for prequalification indicated below. Prequalification's submitted after the deadline will be processed but a bid from that firm will not be opened for any project where the deadline has been exceeded. The following prequalification is required for this contract: TWO prequalification methods are acceptable forthis project. EITHER Bureau ofAeronautics "Statement of Bidder's Qualifications and Experience" (AKA: "Short form") will apply. OR the Bureau of Project Development "Prequalification Statement" (AKA: "Highways"). If your firm does not already hold a valid WisDOT highways prequalification and you do not expect to bid WisDOT highways projects in the future, please complete the simplified Bureau of Aeronautics "short form". A bid security in the amount of 5% of the total amount of the bid is required for the contract. Each proposal must be accompanied by a bid bond, certificate of annual bid bond, a certified check, bank's draft, bank's check, or postal money order made payable to the Wisconsin Department of Transportation Secretary for 5% of the total amount bid. Questions arising from site visits shall be submitted by email to the contact person below. Answers will be provided to all plan holders via a subsequent addendum.
