Contract #1 Item Number - 1 Quantity - 1045 Unit - SY Description - Construct Double Bituminous Seal Coat as per PennDOT Specification (MS-0340-0005) Contract #2 Item Number - 1 Quantity - 160 Unit - TNS Description - Leveling 90 lb/sy 9.5mm 0-.3 ESALS PG-64-22 SRL "M" Item Number - 2 Quantity - 290 Units - TNS Description - Wearing 1.5" 9.5mm 0-.3 ESALS PG-64-22 SRL "M" Contractor required to Contact Municipality for Pre-Bid Meeting Bituminous Price Adjustment as Per Pub 408 Section 110.04 Bidders required to be PennDOT prequalified for work specified.. Proof of Prequalification required with bid submittal. Liquidated damages apply at the rate of $ 800.00 per calendar day. Proposals must be upon the forms furnished by the Municipality. For third class cities the bid must be accompanied by a certified check or bid bond in the amount of 10% of the bid, made payable to the municipality. For other municipal types it is the discretion of the municipality. A performance bond or certified check in the amount of 100% of the contract shall be furnished by the successful bidder within 20 days after the contract is awarded.
