Required for DCAMM contracts over $150,000, Highway Division contracts over $50,000. The City of Revere, Department of Public Works, is requesting written responses for the furnishing of epoxy flooring at the Revere Public Library as further described in ATTACHMENT A, Scope of Services. All Questions must be submitted in writing and must be received prior to 5:00 P.M. on Tuesday, July 2, 2024. B. Under this proposal, the contractor / bidder hereby agrees to furnish all equipment, tools and labor necessary, as well as all incidentals necessary, to complete the installation of an approved epoxy floor system in the Revere Public Library, which is located at 179 Beach Street, Revere, MA 02151. To include all prep work required, inclusive of the removal and disposal of the existing tile flooring. Removing of any underlayment, mastic, glue, tile, adhesive and / or any other type of floor covering. To remove all debris left during the tile removal. Any further scraping, sanding, leveling, smoothing, cleaning etc. as necessary to properly install the epoxy floor system. Floor to be swept and vacuumed free of dust and debris prior to installation and according to manufacturer's instructions, inclusive of concrete dust. To include supplying and installing an epoxy floor system, complete in place, to be installed in the following coats: 1) Poly Crete Coat 2) Water clear with color chips 3) Water clear 4) Armor top
