Process The City Commission will select the best qualified Respondent. A 3-stepprocess will be used: Step 1 - Each RFP proposal will be reviewed, scored and the scoring along with a recommendation for the award shall be forwarded to the Commission for review and selection. Step 2 - If warranted, the short list of individuals/firms may be asked to prepare a more detailed written proposal with personnel, costs, and/or methods described. Step 3 - If necessary, after reviewing additional materials, candidates may be asked to a brief oral interview. Once a final selection has been made, the City and chosen Respondent will work jointly to redefine a detailed scope of work and a contract for services. G. Content of Responses Responses to this RFP must be complete, timely and submitted in conformance with specifications, including all items below: 1. Proposals shall be sealed and indicate on the outside of the envelope the name and address of Respondent. 2. Respondent shall provide one (1) original and one (1) identical digital version in pdf format on CD or USB flash/thumb drive. The digital version must be an identical copy of the fully signed original submission. All required attachments are to be in 8 1/2 " by 11" format and be clearly legible. 3. Proposals shall include a summary of Respondent's business history, qualifications and experience of providing installation, configuration, maintenance of physical facility surveillance cameras. Shall include: tax identification number and corporate office address; comprehensive description of the Respondent's corporate ownership history identifying and explaining if there is any foreign (non-U.S. Based) ownership of the Respondent; whether Respondent has ever been terminated, fired or replaced on a project; whether any contract awarded by another government agency has been terminated by default or convenience; and if Respondent has ever been involved in any litigation against or from a client (if yes, describe). 4. Copy of license to do business in the State of Florida or Sunbiz report showing company registered as "Active." 5. Copy of any and all professional licenses and certifications as required to perform the services described herein and the professional licenses for each team member. 6. Proof of financial ability to reliably deliver services. Respondent shall include information on the nature and magnitude of any litigation or proceeding whereby, during the past five (5) years, a court or any administrative agency has ruled against the firm in any matter related to the professional activities of the firm. This shall include any class actions and/or industry investigations by the Attorney General, in addition to individual cases. Respondent may be required to provide a financial statement. 7. Detailed pricing shall be provided and separated out for each of the various City facilities/locations listed under Scope of Services. Prices shall be provided for all cameras, card readers, door strikes, server systems, and other major hardware components; cable, patching facilities, bonding and grounding components; construction, installation, mounting, connection, and testing; hardware and software licenses; as built and other documentation; and miscellaneous components as required to create a complete and operational system. Any proposal using a 'Cost Plus or 'Cost Plus a Percentage of Cost" pricing method will not be accepted. 8. Detail Responder's warranty that is in addition to or separate from the manufacturer's standard warranty. Respondent should provide the standard warranty for manufacturers of surveillance cameras and equipment. 9. Proof of general liability insurance and workers' compensation insurance including liability limits. 10. A minimum of three (3) verifiable references from similar scopes of work within the past five (5) years as identified in this RFP. Must include name and type of entity, the contact name, email address and telephone number of the public officer in charge of the contract, and the years in which the services have been provided. H. Prices No proposal shall be withdrawn for a period of ninety (90) days subsequent to the proposal opening without the consent of the City of Hawthorne, Florida. Correction of inadvertently erroneous proposals shall be permitted up to the time of RFP opening. Respondents shall not be allowed to modify their proposals after the opening time and date. I. Investigations The City reserves the right to conduct appropriate investigations into the background, previous experience, and training of any firm on individual under consideration for this RFP. J. Evaluation Criteria The City reserves the right to accept or reject any and all submissions in the best interest of the City and in accordance applicable rules and regulations. In connection with the evaluation, the City may, at its option, invite one more Respondent for an interview at a time and location to be announced and may require the submission of supplemental material intended to substantiate or clarify information previously submitted. Respondents submitting the required criteria will have their proposals evaluated and scored for quality of response, qualifications, and experience and costs. The following represents the principal selection criteria, which will be considered during the evaluation process. Criteria will be weighted based on 100% of total value. Category Scores listed below are the maximum number of points to be awarded per each evaluator. The following information will be taken into consideration during the evaluation process: Experience and Qualifications 30 points Score of Services 30 points Cost of Services 30 points Licenses, Financial History and References 10 points Total Maximum Points: 100 points K. Evaluation and Selection Procedure The Evaluation Committee is comprised of City staff and may include a City Commission Member who will evaluate and rank proposals based on a weighted score point formula. All interested professionals are hereby cautioned not to contact any staff or Commission member of the City of Hawthorne, or any member of the Evaluation Committee. Any and all questions should be made through the City Manager and any attempts to lobby or persuade through any other channels may result in disqualification. All proposals will be subject to a review and evaluation process. It is the intent of the City that all Respondents, who meet the requirements, will be scored in accordance with the criteria established in these documents. The City's evaluation criteria, as identified above, for written proposals will include consideration of, but will not be limited to the following: 1. Responsiveness of the proposal related to the Scope of Services; 2. The ability, capability and skill of the Respondent to perform the contract, including the range of services offered; 3. Whether the Respondent can perform the contract within the time specified without delay or interference; 4. The quality, availability and adaptability of the supplies or contractual services to the particular use required; 5. The character, integrity, reputation, judgment, experience and efficiency of the Respondent; 6. Professional licensure required when service of a skilled nature is required by law to perform such service and/or skill; 7. The quality of performance of previous contracts; 8. The number and scope of conditions attached to the bid or proposal; 9. Responsiveness of client references; 10. Net costs; 11. Local Preference Policy (if applicable). The Evaluation Committee will score the Proposals and make a recommendation for the award. All the Proposals, scoring and recommendation shall be forwarded to the Commission for review and selection. The City shall award to the responsive and qualified proposer whose Proposal is determined to be the most advantageous to the City. Evaluation of Proposals shall be based on the evaluation factors set forth herein and any other relevant information obtained through the evaluation process. The City reserves the right to request additional cost/pricing data to the extent determined necessary for the performance of an adequate cost/price analysis. All proposals submitted in accordance with the requirements of this request will be reviewed for completeness and responsiveness. The award will be made to the responsible bidder whose proposal conforms to the solicitation requirements. The City of Hawthorne, Florida reserves the right to: waive any informalities, irregularities or technicalities; reject any and all proposals which are incomplete, conditional, non-responsive, or which contain additions not allowed for; reject any or all proposals in whole or in part with or without cause; and accept the proposals which best serves the City. No Lobbying All Respondents are hereby placed on notice that the City of Hawthorne Commission, City Employees/Staff (with the exception of the City of Hawthorne City Manger, who is designated to receive requests for interpretations or corrections) are not to be lobbied, directly or indirectly either individually or collectively, regarding this RFP. During the entire procurement process, all Respondents and their subcontractors, sub-consultants, or agents are hereby placed on notice that they are not to contact any persons listed above for such purposes as holding meetings of introduction, dinners, etc., if they intent to submit or have submitted proposals for this project. Any Respondent contacting individuals mentioned herein in violation of this warning may automatically be disqualified from further consideration from this RFP. M. Public Entity Crimes Pursuant to Florida Statutes section 287.133(2)(a), all Proposers are hereby notified that a person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for the construction or repair of a public building or public work, may not submit bids on leases of real property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, and may not transact business with any public entity is excess of the threshold amount provided in Florida Statutes section 287.017 for CATEGORY TWO (currently $25,000) for a period of 36 months from the date of being placed on the convicted vendor list. A "Public entity crime" means a violation of any state or federal law by a person with respect to and directly related to the transaction of business with the State of Florida, any of its departments or agencies, or any political subdivision of the State of Florida, or with an agency or political subdivision of any other state or with the United States, including, but not limited to, any bid or contract for goods or services, any lease for real property, or any contract for the construction or repair of a public building or public work, involving antitrust, fraud, theft, bribery, collusion, racketeering, conspiracy, or material misrepresentation. N. Incurring Costs The City of Hawthorne is not liable for any costs incurred by respondents prior to the contract agreement
