Clay County is requesting quotes from qualified contractors to provide housing rehabilitation services at the following location: 475 Springbrook Drive Orange Park. FL 32073 Below is the list of recommended repairs: Site Work Remove and dispose the three (3) dead trees in proximity to the house on right side. Master Bath Eliminate the master jacuzzi tub due to the homeowners physical limitations. Convert existing tub area (45" x 58") to a walk in shower which shall include but not limited to, relocating electrical, building minimal sill, installation of pan with blocking, installation of duro-rock and installation of tile. Install a new valve and double shower head. Contractor shall relocate the drain if necessary to accommodate the new walk-in shower. The new walk-in shower shall be a unit without tile. Exterior Siding Replace piece of rotten siding off the rear porch (located left of the screen door). Plumbing Repair the counter in the kitchen where it appears there is a leak and the counter top is expanding. Sheetrock Repair Patch over nail pops in the hallway. Apply Kilz and blend with flat ceiling paint over the stained ceiling in the hall bath and adjacent closet. Substantial Completion shall be obtained within sixty (60) days from Notice to Proceed. Final Completion shall be obtained within ninety (90) days from Notice to Proceed. The Contractor shall be assessed Liquidated Damages (LD) of $100.00 for each day past final completion date set by the contract.
