The items requested within this RFP may or may not be USF eligible. For guidance, all non-eligible products and services must be itemized individually based on the USF Eligible Services List. (Note: The vendor checks for updates and corrections before submitting bids/quotations.) Please submit the RFP and three K12 educational References if you have not previously worked directly with Mecosta-Osceola ISD. Please list in your proposal all the Service Level Agreements (SLAs) of the services offered Please list in your proposal any user privacy policies for data All bidders must adhere to the Lowest Corresponding Price (LCP) guidelines (as the SLD dictates). All bidders understand that if the entity closes, contract termination fees will not be incurred by the entity. All bidders understand that the entity may choose multiple providers for one type of service. Multi-year contracts may be considered. All bidders must have an E-rate SPIN Number All bidders must have current Service Provider Annual Certification Form 473 on file. The implementation and completion of the services are contingent upon the district receiving approved funding from the Universal Services Fund. The services may be revised or terminated if the funds from the USF discount program are not approved or do not meet the entity's expectations. If requested, the vendor will complete the SPI (Form 474) application. The vendors are responsible for understanding and complying with all rules and procedures required by the Schools and Libraries Corporation and the Universal Services Fund.
