All submissions shall contain Indiana statutory Form 96, including, without limitation, financial statement, no collusion affidavit, and certification of non-segregated facilities of the bidder. Each bidder must comply with IC 22- 5-1.7 et seq. and participate in the E-Verify Program and execute an affidavit of compliance. Each bidder must ensure that all employees and applicants for employment are not discriminated against because of race, creed, colour, sex, sexual orientation, disability, or national origin. No bidder may withdraw a bid within a period of 60 days following the date set for receiving bids. A certificate of insurance naming the Town of Beverly Shores as an additional insured shall be required from all contractors who are awarded a bid before they can commence work and must contain the minimum acceptable insurance limits of $250,000 property damage, $750,000 per person for bodily injury or death with a combined single limit of $1,000,000 per occurrence, and limit of $2,000,000 general aggregate. The Town of Beverly Shores reserves the right to waive any formality, informality, or irregularity in any bid received, and also reserves the right to reject any and all bids and to re-advertise for bids.
