The City of Toledo Dpu/sewer and Drainage is Seeking Quotes for Bulk Supply of Sand and Stone. All Bid Items Shall Meet the Current Requirements of the Ohio Department of Transportation (Odot), Currently 2019 Construction & Material Specifications. As noted above, City forms must be filled out and attached to the bid. Project Paperwork includes the following: Acknowledgment and Acceptance of Conditions Contract Specifications; Toledo Municipal Income Tax Compliance; Public Utilities Compliance; Living Wage Affidavit; Living Wage Contract Addendum; EEO Form; Primary and Subcontractor Forms and Debarment and Suspension Certificate; Best Bid Criteria.
