Work will be performed at various locations in the Borough of Pottstown area for Street Overlay of approximately 6,000 Tons. Work will include supplying and installing Asphalt Mixture Design 9.5 mm, WC PG 64S-22, .3-30 M ESAL'S, SRL-H, sealant, with approximately 52,000 Syds of milling and approximately base replacement as needed Item 2: Alternate Bid: An alternate list of additional streetsmay be considered by theBorough of Pottstown as anaddition to the contract. Thealternate list of streets maybe selected as an individualstreet, several streets, or allstreets on the list to be addedto the contract. Each street onthe list shall be priced as unitprices as per Base bid Tab 1Paving and 2. Milling. An ORIGINAL bid bond, certified check or cashier's checkdrawn to the order of theBorough of Pottstown in theamount of Ten Percent (10%)of the bid price must be DELIVERED, separate from theonline bid, to the Office of theBorough Manager, Boroughof Pottstown, Borough Hall,100 E. High Street, Pottstown,Pennsylvania 19464 prior tothe bid due date and time asBid Security, which will be returned after the Award of theContract. All Bidders are required tosubmit the following documents with their bid submission: 1. Bid Proposal form, 2.PennDOT MS-944 (Proposal &Contract) Form 3. Non-Collusion Affidavit and Instructions, 4. Public Works Employment Verification Act, Act 127of 2012 shall be accompaniedwith the Public Works Employment Verification Form.,and 5. Statement of Qualifications. The successful Bidder is required to submit the followingcompleted documents to theBorough.1.
