Lead-Based Paint Hazard Remediation and Construction Services (LHR-019)" for the properties located at 1. 3811 1/2 Flower Street, Cudahy, CA 90201 2. 3813 1/2 Flower Street, Cudahy, CA 90201 3. 3813 1/4 Flower Street, Cudahy, CA 90201 This will include furnishing all labor, material and equipment, and performing all work necessary and incidental in accordance with Appendix B - Scope of Work Questions regarding the RFQ are to be directed to Jackie Reyes at (626) 586-1789 or by email at jackie.reyes@lacda.org The Work to be performed under this Construction Contract shall be commenced within ten (10) days after a Notice to Proceed is received by the Contractor, or on the date specified in the Notice, whichever is later, and shall be completed within Thirty (30) calendar days following the required commencement date. The Contractor and the LACDA agree that, since the determination of actual damages for any delay in completion would be extremely difficult or impracticable to determine in the event of breach of contract, the Contractor and his/her sureties shall be liable for, and shall pay to the LACDA the sum of Five Hundred Dollars ($500.00) as liquidated damages for each calendar day of delay, until the Work is accepted by the LACDA. The Contractor shall not be charged with liquidated damages because of any delay in the completion of the Work due to unforeseeable causes beyond the control and without the fault ornegligence of the Contractor. The LACDA reserves the right to reject any and all quotes. This RFQ is not a contract or commitment of any kind. The LACDA is not liable for costs incurred in the preparation of the quotes submitted by contractors. It reserves the right to issue supplementaryinformation or guidelines related to this RFQ. Notwithstanding any other provisions herein, the LACDA reserves the right in its sole discretion to waive minor technical deficiencies in the quotes. In accordance with the Civil Rights Act of 1964, Americans with Disabilities Act of 1990, Age Discrimination Act of 1975 and Section 504 of the Rehabilitation Act of 1973, it is the policy of the LACDA to assure equal opportunity to all persons, in the award and performance of any contract, without regard to race, color, sex, religion, national origin, ancestry, age, marital status, or disability
