Provide all labor, equipment, and materials necessary to provide tempered/hot water systems to fourteen emergency shower/eyewash locations--nine systems at the Fleur Drive Water Treatment Plant and five at the McMullen Water Treatment Plant. Some locations have existing combination shower/eyewash fixtures, while other locations require either combination fixtures or replacement of portable units. System upgrades will include modifications to the existing plumbing and electrical systems to produce the required amount of tempered water supply for the emergency fixtures. The construction of the improvements shall be in accordance with the project plans and specifications and general stipulations for said improvements as approved by the Board. Instructions to bidders include: - Bid must be accompanied by a bid bond or other security permitted by statute in an amount equal to ten percent of the amount bid, in a separate sealed envelope identified as Bid Security. - The proposal must be in a sealed envelope identified as the Proposal with the contract title and the name and address of the bidder. - Declaration of Non-Collusion and Resident Status and Non-Discrimination Clause must be completed and signed. - Proposals must be correctly filled out, including acknowledgment of any applicable addenda, and signed with address completed. - Use the preprinted form furnished by the Board for bidding. - Any bid received after the scheduled closing time will be returned unopened and will not be considered. - Each bid must be accompanied by a cashier's or certified check drawn on a bank in Iowa, a certified share draft, or a bid bond, for the required amount, in a separate sealed envelope. - Checks, share drafts, and bid bonds will be returned to unsuccessful bidders. - The successful bidder must furnish a contractor's bond equal to one hundred percent of the contract price, guarantee materials and workmanship to be free of defects for two years, and guarantee surface restoration work in public right-of-way for four years. - Liability insurance certificates and all required documentation must be delivered prior to commencement of any operation. - Proposals not submitted on the forms furnished, or with irregularities, conditions, alterations, or omissions, are subject to rejection. - Entities organized outside Iowa must file a certificate with the Secretary of the State of Iowa and provide proof of proper qualifications and licensure. - Proposals may be rejected if there is evidence of collusion, default on a previous contract, or other disqualifying conditions. - The Board reserves the right to reject any or all bids and to waive informalities in any bid.
