The project is for sitework, utilities, and the relocation/alteration of three portable classroom buildings from Paradise Ridge Elementary School to the Honey Run Academy site. The scope includes all work associated with the relocation and alteration, as well as inspection requirements as detailed in the DSA-103 form for a Class 3 Project Inspector (PI). Proposals must include a not-to-exceed fee, which should break down hourly rates, estimated number of hours, and a total not-to-exceed amount, with a 10% contingency. All anticipated project expenses, including deliverables and travel, are to be included in the proposed fee. Proposals shall not contain exclusions that would be required to complete all inspections. Additional services for the normal completion of the requirements will not be accepted, except in cases of unforeseen conditions discovered after a thorough field visit; these must be proposed in writing to the architect for approval before commencing work. Proposals must also include evidence of insurance as specified: $1,000,000 per occurrence and $2,000,000 aggregate of Commercial General Liability, $1,000,000 per occurrence of Automobile Liability, Workers' Compensation Insurance in compliance with statutory requirements and $1,000,000 Employer’s Liability, and if applicable, Professional Liability coverage of $1,000,000 per claim/$2,000,000 per occurrence. By submitting a proposal, the firm agrees to all stated terms and requirements, and the selected party will be required to enter into an Independent Contractor Agreement with Paradise Unified School District. A DSA approved Class 3 Project Inspector is required for this project. Reference documents including drawings, specifications, and DSA 103 are made available for review and must supplement the proposal.
