The award will be made to the responsible vendor submitting a conforming proposal meeting specifications at the lowest cost unless other criteria are noted in the Request for Proposals. Unless otherwise noted, the award may be made by individual items. If there is a discrepancy between the unit price and the extension, the unit price will prevail. Discounts will not be considered in making award but may be offered on the invoice for earlier payment and will be applicable on the date of completion of delivery or receipt of invoice, whichever is later. Scope of Work The successful contractor shall supply all equipment, materials and labor required to remove and dispose of old flooring (where necessary), clean and prep areas for new flooring, and supply and install new flooring as required and as recommended by manufacturer. Areas of work include Entry way, Two Bathrooms, Closet Spaces, ADA Bathroom, Main Space (Including Stage), Two Storage Spaces off Main Space and Kitchen Space with Storage Closets. 1. Toilets to be pulled and placed offsite. 2. Kitchen Items such as stove, Large Island on wheels and refrigerator will move around to install. 3. All Floor Grates will be removed and sandblasting and repainting prior to reinstallation. 4. Use existing Metal Thresholds 5. Remove existing Carpet on stage and prep for new flooring. 6. Remove existing four inches (4") vinyl base and prep for new four inches (4") vinyl base, Brown 7. Install new carpet on raised stage: Ancestry Color Patriarch - Broadloom commercial carpet 30oz Nylon Carpet 8. Install new .25" plywood over existing VCT flooring in the above spaces. 9. Install new Tarkett II VCT (Vinyl Composition Tile) over new plywood subfloor in areas listed above. 10. Post installation seal and waxing of new flooring. The City of Laconia reserves the right to accept or reject any and/or all proposals or any part thereof, to waive any formality in the process, and to accept the proposal considered to be in the best interest of the City. Failure to submit all information called for may be sufficient grounds for disqualification. The City of Laconia reserves the right to cancel this request at any time for any reason. Proposal Requirements Three (3) copies of a proposal for installation services should be delivered to the Purchasing office, 45 Beacon Street East, Laconia, NH 03246 by 2:00pm, February 12, 2024. The proposal should include the following: 1. A description of the firm and its related experience in similar projects. Include appropriate summaries of projects as well as names and telephone numbers of references that may be contacted. Proposals should include a minimum of three (3) references from customers who have contracted similar services. 2. A description of the project approach and schedule. 3. A not to exceed cost inclusive of all charges and expenses for the project including the scope of work outlined in this request for proposal.
