Molino Avenue Safety Improvements. Project Description: The work generally consists of mobilization, traffic control, roadside concrete swale demolition and removal, installation of new 12" storm drain, installation of 2 drain inlets, modifying 1 existing drainage inlet, installation of new curb & gutter, and patch hot mix asphalt pavement, replace traffic striping, clean-up of the project area, and other items of work as shown on the plans. The project is located near 437 Molino Avenue in the City of Mill Valley. Project is to be completed within Twenty (20) consecutive working days from the effective date of the Notice to Proceed for the base bid. A Bid Bond in the amount of 10% of the total bid amount payable to the City of Mill Valley shall accompany the Proposal. Award of contract, if awarded, will be to the lowest responsible bidder whose proposal complies with prescribed requirements, and will be within thirty (30) days after receipt of proposals. Telephones will not be available to bidders for the preparation of bids or for calling in bid results. Bid forms received after the designated time shall not be accepted. No bidder may withdraw its bid for a period of sixty (60) days after the date set for the opening of bids. PROPOSAL GUARANTY: All bids must be submitted on the proposal Form obtained from the Department of Public Works and shall be accompanied by a Proposal Guaranty of at least ten (10%) of the base bid. Guaranties shall be in the form of cash, a certified or cashier's check or Bid Bond payable to the City of Mill Valley. Failure of the successful bidder to execute and return the contract or to file acceptable bonds, as required, within the time allotted shall be cause for the annulment of the award and forfeiture of the Proposal Guaranty. The Performance Bond shall be in an amount equal to one hundred percent (100%) of the total amount bid by the Contractor in his proposal. The Labor and Materials Bond shall be in an amount equal to one hundred percent (100%) of the total amount bid by the Contractor in his proposal. The City reserves the right to reject any or all bids, and to waive any irregularities in the bids. The proposed scope of work includes installing approximately 201 linear feet of 12in RCP storm drain pipe along Molino Avenue in the City of Milly Valley. As outlined in the schedule of bid items, this work includes, but is not limited to, mobilization, traffic control, installation of new 12in storm drain, installation of 2 new drain inlets, modifying 1 existing drainage inlet, installation of curb & gutter, hot mix asphalt pavement patching, replacing traffic striping, clean up of project area, and other items of work as shown on the Plans, as called for in the Special Provisions, and as directed by the Engineer.
