Furnishing all tools, materi- als, and labor and performing the work necessary to reconstruct approximately 650 linear feet of existing residential brick street and 300 linear feet of existing asphalt with new asphalt pavement. Work will in- clude 2200 linear feet of new concrete curb and gutter, 700 square yards of new concrete sidewalks, 730 square yards of new concrete driveways, and approximately 545 tons of new asphalt pavement. This project also includes furnishing all tools, materials, and labor and performing the work necessary to be done to construct 1010 linear feet of new 6 inch PVC water line to relocate and replace existing deteriorated 6 inch water line that lies under the street being reconstructed. In order that the Owner may know to whom further information, addendums, and clarifications should be sent prior to the bid opening it is required that the plans and specifica- tions shall be obtained from the Office of the City Clerk, 516 East Grand Avenue, Ponca City, Okla- homa, 74601 in order for a contractor to be eligible to bid on this project. The information for Bidders, Forms of Bids, Form of Contract, Plans, Specifi- cations and Form of Bid Bond, Performance and Statutory Bond, and other contract documents may be examined at the Office of the City Clerk of Ponca City, Oklahoma. The successful bidder shall submit Certification of Insurance for Public Liability and Workers Compensation in the amounts prescribed by Statute with the City of Ponca City named as additional insured. Bids filed as provided herein shall be publicly opened on the above-men- tioned time, date and place. All bids shall remain on file at least forty-eight (48) hours thereafter be- fore a contract shall be made and entered into thereon. Bids received more than ninety-six (96) hours, excluding Saturdays, Sundays and holi- days, before the time set for the opening of bids, as well as bids received after the time set for opening of bids, shall not be considered and will be returned unopened. All bids submitted must include the word "BID", the name of the bid item or project, and the time and date of the bid opening on the face of the sealed envelope. A cashier's check, a certified check or a security bond in the amount of five percent (5%) of the bid shall accompany the sealed proposal of each bidder. Deposits will be returned to unsuccessful bidders.
