Each bidder is required to furnish with its proposal, a Bid Guaranty and Contract Bond in accordance with Section 153.54 of the Ohio Revised Code. Bid security furnished in Bond form, shall be issued by a Surety Company or Corporation licensed in the State of Ohio to provide said surety. The project will consist of 2.0" of asphalt concrete resurfacing to correct failures throughout the system throughout the village's roadway system. The work includes the reconstruction of existing roadway cross-slope to improve drainage characteristics along with the adjustment or reconstruction of the existing curb inlets and manholes that are in deteriorated and non-functional condition. Included as part of the project will be the construction of ADA compliant curb ramps as well as the replacement of a small section of concrete curb. The work will take place in the following locations: Utah Avenue (W. 6 th St. to Dakota Ave.), Dakota Avenue (Utah Ave. to Nebraska Ave.), and Nebraska Avenue (W. 5 th St. to W. 3 rd St.). Each Proposal must state full name of the party or parties submitting the proposal and all persons interested therein. Each bidder must submit evidence of its experiences on projects of similar size and complexity. The owner intends and requires that this project be completed in 30 calendar days from the notice to proceed All contractors/subcontractors involved with the project will, to the extent practicable use Ohio products, materials, services, and labor in the implementation of their project. Additionally, contractor compliance with the equal employment opportunity requirements of Ohio Administrative Code Chapter 123, the Governor's Executive Order of 1972, and the Governor's Executive Order 84-9 shall be required. Bidders must comply with the prevailing wage rates on Public Improvements in Trumbull County and the Village of McDonald, Ohio as determined by the Ohio Department of Commerce, Bureau of Wage and Hour Administration, 614.644.2239. No bidder shall be considered a competent and responsible bidder or eligible to be awarded the contract to which this Notice applies, if the bidder is listed on the Auditor of State's database as having a "Finding of Recovery" as that term is defined in O.R.C. 9.24. Bids shall be submitted on forms provided and shall state the Unit Price or Lump Sum as indicated by the blanks provided therefore; and shall be enclosed in a sealed envelope marked on the outside "VILLAGE OF McDONALD - UTAH, DAKOTA, NEBRASKA RESURFACING" with the contractor's name, address, phone number, email address and contact name. The Village of McDonald reserves the right to reject any or all bids and to waive any formalities or irregularities in bidding.
