Bemidji Township (the "Township") invites written Quotes from qualified bridge-maintenance contractors to perform priority repairs at Bridge #04520, located on Yellowhead Road (Township Route 998) over the Mississippi River. The work is strictly maintenance; no structural modifications are required. 2. SCOPE OF WORK - PRIORITY TASKS 1. Priority 1 - Remove and dispose of approximately 3,000 square feet (+-30 %) of damaged or settled grouted rip-rap. 2. Priority 2 - Install new Class I grouted rip-rap to original grades and re-grout all joints to prevent water intrusion. 3. Priority 3 - Drill exploratory holes through the concrete approach slab to identify any sub-slab voids. 4. Priority 4 - Pressure-grout detected voids to restore full bearing under the approach slab. All materials and workmanship shall comply with the 2020 MnDOT Standard Specifications for Construction. 3. PERMITS Contractor shall be responsible for any/all permits related to, or necessary to complete the work including but not limited to work adjacent to water, erosion control, DNR etc. 4. TRAFFIC CONTROL A full closure of Yellowhead Road at the bridge site is allowed for up to 48 consecutive hours. The Contractor shall develop and implement a Temporary Traffic Control Plan in accordance with the Minnesota Manual on Uniform Traffic Control Devices (MUTCD), including advance warning signage and local detour routing. Traffic control devices shall be maintained at all times during the closure. Site Directions: Yellowhead Road (Township Route 998) over the Mississippi River. 5. SCHEDULE The Township anticipates issuing a Notice to Proceed in August 2025. All work, including site restoration, must be substantially complete by **October 22, 2025** 6. AS-BUILTS NARRATIVE AND DOCUMENTATION Prior to completion of the final inspection, the Contractor shall provide to Owner an as-built narrative and documentation of all work completed in the maintenance and Repair of Bridge #04520. 7. CONTRACTOR QUALIFICATIONS Completion of at least two bridge rip-rap or grout maintenance projects within the last five years. Demonstrated ability to pressure-grout approach slabs and manage riverfront erosion control. MnDOT Pre-qualification Work Type 5.51 (Bridge Deck Rehabilitation) or equivalent experience.
