Replacement of sections of roofing at North Daviess Jr./Sr. High School as detailed in the Construction Documents. Bids are to be properly and completely executed on Indiana State Board of Accounts, Form 96, Revised 2013, and supplemented by the forms included in the contract documents. Submit all original bid information and two complete copies, including completion of the entire Bid Form(s), Non-Collusion Affidavit, and Bidder's Financial Statement information. Bidders are required to include a Bid Bond or certified check in the amount of 10% of their Bid Price. The awarded bidder must provide a Performance Bond and Labor and Materials Payment Bond in the amount of 100% of the contract. All costs for bonds and insurance are to be included in the bid. Each bid envelope must be marked "Roofing Bid: North Daviess Jr./Sr. High School" with the name of the Roofing Contractor and Project Number clearly indicated. Each bid must also include a signed and completed Non-Discrimination Affidavit (or MBE-1) in accordance with applicable Minority Business Participation Policy and Rules. Faxed or emailed bids will not be accepted. Bids must remain effective for 60 days. The contract will be awarded to the best responsive and responsible qualified bidder(s). The North Daviess Community Schools reserves the right to reject any and all bids, is not obligated to accept the lowest or any other bid, and may waive any formalities in bidding procedure.
