The work includes, but is not limited to, excavation and removal of existing concrete abutment walls, embankment excavation, reinforced concrete box culvert installation, and reinforced concrete wing wall installation. Bidders must be properly licensed in the City of Crown Point at the time bids are received. All bids must be executed on the forms provided in the Contract Documents. Each bid must be accompanied by a certified check or cashier's check, or a bid bond of at least five percent (5%) of the total bid price from an incorporated surety company qualified in Indiana. A performance bond and payment bond, each for one hundred percent (100%) of the contract price, will be required. Contractors must use Standard Indiana Form 96, including a non-collusion affidavit, and must subscribe to the Indiana Plan and an apprenticeship and training program if their bid is accepted. Proof is required before any payments are made. Bidders, contractors, subcontractors, and suppliers must have at least five (5) years of progressive experience in similar work. Any bidder with outstanding fees, inspections, or unresolved work/quality issues with the City of Crown Point may be deemed non-responsive or non-responsible. Bidders must not discriminate in employment practices, must comply with the City's Responsible Bidding Ordinance, and must participate in the E-verify program. Certification of non-collusion and no investment activities in Iran must be submitted. The successful bidder must provide insurance certificates naming the City and its professional service agents as additionally insured before the project starts and execute a formal agreement with the City.
