This work shall include, but is not limited to, the installation of decorative lighting (poles, foundations, wiring) as well as the removal and replacement of sidewalks and ADA Curb Ramps, within the project limits, in the City of Hammond, Lake County, Indiana. All Bidders must be properly licensed in the City of Hammond at the time bids are received. All Bidders must comply with Hammond Ordinance No. 9519 related to Responsible Bidding Practices that requires all Bidders to become prequalified by the City of Hammond Building Commissioner prior to submitting a Bid for Public Works for the City of Hammond. Each bid shall be accompanied by an acceptable certified check or cashier's check made payable to the City of Hammond or an acceptable Bid Bond in an amount not less than five percent (5%) of the total bid price executed by an incorporated surety company in good standing and qualified to do business in the State of Indiana. A performance bond and payment bond each in the amount of one hundred percent (100%) of the contract price will be required. Standard Indiana Form 96 (Revised 2013) which includes non-collusion affidavit will be required. Contractors submitting a bid for City construction projects must subscribe to the Indiana Plan and a joint apprenticeship and training program for that project if their bid is accepted and provide proof of same before any payments are made. Each bidder must ensure that all employees and applicants for employment are not discriminated against because of their race, creed, color, sex or national origin and must conform with the anti-discrimination provisions of IC 5-16-6. An Affirmative Action Statement must be submitted at time of bid.
