Box 100 PO Box 100 Wellpinit, WA. 99040 Wellpinit, WA. 99040 The Spokane Tribe is seeking proposals from qualified contractors to perform a full depth reclamation of an existing asphalt access road and extend the length of the existing road. Services include cleaning and vegetation removal, asphalt patch repair, crack sealing, asphalt milling, application of new asphalt on milled areas, subgrade preparation, and installation of new asphalt access road per the contract drawings. Proposals must include: 1. Pre-proposal site visit with Spokane Tribal Hatchery Manager to view existing conditions and gather information needed for a total cost proposal that meets all technical specifications and requirements listed in Attachment - Spokane Tribal Hatchery Access Road Repair Project Drawings and Technical Specifications. 2. Total cost, with breakdowns of all services and products required to complete the full scope of work as listed in Attachment B. This breakdown must include administrative costs for Spokane Tribal Employment Rights Ordinance, prevailing wages in compliance with Davis Bacon Federal rule, and bonding cost for the full value of the cost proposal. 3. Proposed project timeline. 4. Proof of insurance and bonding. 5. A copy of the RFP with certifications via vendor signature regarding debarment/suspension (see Attachment A), and a signature certifying that the total cost proposal is accurate and valid for vendor selection and contract award.
