Site work and paving for a mixed-use development in Bellows Falls, Vermont. Completed plans call for site work for a water / sewer project; sidewalk / parking lot; for paving for a sidewalk / parking lot; and road / highway.
GENERAL CONSTRUCTION NOTES o All material to be installed in accordance with manufacturer's specifications and instructions. o The State of Vermont specifications shall be adhered to, a summary of some of the pertinent ones are attached as Additional Specifications. Any reference to "Agency" or TOWN shall imply the Town of Rockingham. o Notify Digsafe at least 72 hours prior to construction.3 o It shall be the contractor's responsibility to comply with OSHA and VOSHA requirements, maintain a safe job site, and protect the safety of the public. o Town will stake out the North side of the road and proposed retaining wall with offset stakes. Warranty The Chosen Contractor Shall Warranty Their Work to Be Free From Defects in Material and Workmanship for a Period of One Year From Substantial Completion. Insurance Requirements 1. The Contractor Shall Purchase and Maintain Such Insurance as Will Protect Them From Claims Set Forth Below Which May Arise Out of or Result From the Contractor's Operations Under the Contract, Whether Such Operations Be by Itself or by Any Sub-contractor or by Anyone Directly or Indirectly Employed by Any of Them or by Anyone for Whose Acts Any of Them May Be Liable. o Claims Under Worker's Compensation Disability Benefit and Other Similar Employee Benefit Acts; o Claims for Damages Because of Bodily Injury, Occupational Sickness or Disease, or Death of His Employees, and Claims Insured by Usual Personal Injury Liability Coverage; o Claims for Damage Because of Bodily Injury, Sickness or Disease, or Death of Any Person Other Than His Employees, and Claims Insured by Usual Personal Injury Liability Coverage; and o Claims for Damage Because of Injury to or Destruction of Tangible Property, Including Loss of Use Resulting There From. 2. The insurance required by the above sub-paragraph shall be written for not less than the following minimum limits of liability: o Worker's Compensation: Statutory ($500,000/$500,000/$500,000) o Comprehensive General Liability: Bodily Injury: $1,000,000/$1,000,000 (Each Person/Each Occurrence) Property Damage: $100,000/$300,000 (Each Occurrence/Aggregate) o Comprehensive Automobile Liability: Bodily Injury: $1,000,000/$1,000,000 (Each Person/Each Occurrence) Property Damage: $500,000 (Each Occurrence) 3. Prior to commencing work, Contractor shall provide the Town with a Certificate of Insurance and amendatory endorsement naming the Town as an Additional Insured on the liability coverages. Upon receipt of these two bonds, the Town will issue a Purchase Order to the Chosen Contractor. A Bid Bond is required with the bid submittal.
