If you have any questions about the RFQ, please contact Taylor Hooker at 423-476-0620 ext. 2043 or Kyle Page, 423-715-8840. pplicable to the scope of work. o Mobilization o Site Demolitiono Base Stone o Asphalt Binder (64/22 BM2)o Asphalt Topping (64/22 411E) o Modification to utility and drainage structureo Striping o Handicap Markingo Milling (minor modifications around drainage profiles)o Prime Coato Tack Coato Traffic Controlo Additional Materials (include a description of each)o Contingency (include description & specific notes)o Performance & Payment Bond The proposal must include the following items of work: Mobilization; Site Demolition; base Stone Placement for Proposed Asphalt Paving; Minor Modifications to Certain Water and Sewer Utility grating profiles, all in accordance with the scope of work as noted below1. Asphalt Paving & Associated Scopes: a. Overlay of existing pavement. Place 1.5" of 64/22 Grade E Hot Mix Asphalt. Apply & Tack Coat to existing surface for adhesion (.03-.05 gal/sy). Restripe any existing pavement markings that are affected during the overlay process. Pavement markings shall be applied with latex traffic-grade paint. 2. New Asphalt Paving a. Asphalt paving section shall be 6" Mineral Aggregate Base, apply prime coat to aggregate base (.3 gal/sy), then install 2" of 64/22 Grade BM2 (Binder), and 1.5" 64/22 Grade E (Surface). Apply Tack Coat between the binder and surface layer for adhesion (.03-.05 gal/sy) Listed below are additional calcifications and exclusions - including, but not limited to: o Payment and performance bond is not included in the proposal. EARTHWORK: o Grading tolerance for soil subgrades assumed at +/- 0.1', all areas. Additional General Notes for Parking and Paving Installation o All work shall comply with all applicable codes, regulations, and/or local standards imposed by the local utility, city, county, and state. It is the contractor's responsibility that all the construction be in accordance with Bradley County and TDOT Standard Details and specifications. o Contractor shall comply with all pertinent provisions of the "Manual of Accident Prevention in Construction" issued by AGC America, Inc. and the safety and health regulationsfor construction, as issued by the U.S. Department of Labor o Contractor shall provide all provide and maintain all Safety devices, lights, barricades, warning signs, and guards as necessary for the protection of the work area and surrounding property. o The contractor shall take necessary measures to separate work areasfrom pedestrian traffic and to always ensure safe pedestrian passage. o All signs, pavement markings, and other traffic control devices shall conform to the manual on uniform traffic control devices. A minimum clearance of two feet shall be maintained between the face of curb and any part of a traffic sign or light pole. Contractorshall coordinate installation of all signs, pavement markings, and other traffic control devices with other contractors on signs or light poles. o Contractor shall furnish and install pavement markings as shown. All pavement markings shall be four (4") wide solid white unless indicated otherwise on drawings and project documentation. Paint shall be applied in two coats for a minimum dry film thickness of 15 mil. Thermoplastic striping, if required, shall be spray type, applied in accordance with DOT standards. o Contractor shall saw-cut to provide smooth transitions at tie-ins to existing edges of pavement and at cold joints of recently paved asphalt. Joints or score marks are to be sharp and clean without showing the edges of the joining tool. The contractor shall saw cut tie-ins at existing curbs as necessary to ensure smooth transitions. The contractor shall saw cut and transition to meet existing pavement as necessary and as directed by the inspector to ensure positive drainage. o Paving contractor shall install paper breakaway edges at cold joints or saw cut as required to insure a straight, full-depth joint face immediately prior to installing abutting hot asphalt. o Asphalt surface course shall be laid with the direction of traffic in all drive lanes within parking fields o Base and asphalt thicknesses are the minimum required. Refer to the project specifications above for the type of paving and base to be used o The Contractor shall provide on the job site ample and suitable containers with covers, and shall be fully responsible for containing and removing from the site all refuse from meals eaten on the site and other rodents or vermin-attracting refuse. o These Specifications establish construction and material standards and techniques and do not necessarily cover all specific items of materials required for a complete installation
