Cold Spring Community Center Flooring Project All questions are to be submitted in writing via email to: Tim McDavitt, Purchasing/Finance tmcdavitt@northkingstownri.gov, NO LATER than December 15, 2025, at 4:00pm in order to post any necessary Addendum in a timely manner. The Town of North Kingstown (Owner) is seeking bid proposals from qualified contractors for work associated with improvements to the North Kingstown Cold Spring Community Center building. located at 10 Beach Street, North Kingstown, RI. o Review of the existing conditions, field measurements. (Refer to Attached Floor Plan for Areas of Flooring to be installed). Areas shown are estimated, it is the Bidder responsability to verify quantitties. (Removal of the existing VCT will be performed by NK DPW facilities personnel.) o Mechanically clean the floor to a surface profile of 3. o Apply a Moisture Mitigation system to the existing slab and apply a skim coat (Self Leveling) over the existing concrete floor. o Furnish and install a 5mm vinyl plank (see plan and LVT specs), cemented direct (adhesive must work with Moisture Mitigation System and/or Self Leveling Material). Color to be selected. o WALL BASE N.I.C. (Not in Contract). o Furnish and install a 15.20x15.20 cm (6"x6") quarry tile and Quarry Cove Base with inside and outside corners are required (see plan and tile specs), cemented direct (adhesive must work with Moisture Mitigation System and/or Self Leveling Material). o It is the Contractor's responsibility to ensure that installation meets all current building code and flooring product manufacturer's requirements (code and manufacturer's requirements). o Attached RFP Specifications must be referenced for the selection and installation of project materials, execution and administration of the work, quality assurance, regulatory requirements, and warranties. o The Owner must be made aware of all product substitutions, which must be dimensionally accurate and equal or better in all respects to those specified. It is the Contractor's responsibility to provide all information necessary to accurately determine and justify any added work to the scope of work. (It is the Bidder responsability to verify quantitties). o Firms and individuals involved with this project must be identified on the Bid Form. All credentials, licenses, relevant work experience and general information in support of the Selection Criteria contained in this RFP must be listed with this RFP submission in order to be considered in the Town's RFP evaluation and recommendation of the award to the Town Council. o All Contractors shall be licensed in the State of RI for all trades required by this RFP and will not be allowed to sub-contract any of the services required without prior authorization from the Town. It is the responsibility of the Contractor to ensure that any additional subcontractor work and subsequent costs associated with subcontract work receive prior authorization by the Town. Copies of Contractor's licenses must be submitted with this RFP submission. Failure to maintain a Rhode Island Licenses(s) and/or insurance coverage is grounds for immediate termination. o Contractors must conform to RI Prevailing Wage requirements,set forth in RIDLT Municipal Contract Addendum, Prevailing Wage Requirements (RIGL 37-13-1 Et Seq). Information can be obtained at the following website: http://www.dlt.ri.gov/pw Contractor is fully responsible for compliance. o The Town of North Kingstown shall hold the Contractor liable for all damage caused by his employees to any Town equipment, apparatus, existing or installed property and buildings. The Contractor shall procure, at his expense, all necessary insurance, licenses and permits and shall conform to all laws and regulations. o The Contractor shall secure and make safe the entire construction site area to the public.
