DESCRIPTION OF WORK: This work is for Phase 1 of a greater plan to create ADA Accessible pathways from Gayley Road up to Stern Hall and Foothill Dining Hall with lighting and landscaping. Repaving of existing pathways and steps inside limit of work, including demolition of existing pathways, to improve access to Stern Hall and Foothill Dining Hall. Improvements are being made to upgrade the aging infrastructure, provide an improved ADA accessible route, and re-establish irrigated vegetation on the sloped site. BIDDER QUALIFICATIONS: To allow the University to evaluate bidders, each bidder must submit with its bid, a Contractor's Special Statement of Experience. This Contractor's Special Statement of Experience must be provided on the Contractor's letterhead. If your firm provides inaccurate or incomplete information your firm will be found non-responsive. If your firm receives negative project references they may be found non-responsive. To be eligible for consideration of award, the Contractor's Special Statement of Experience must include: 1. Documentation that the bidder has successfully completed as prime contractor, the following: a. A minimum of three (3) and a maximum of five (5) comparable ADA Accessible paths and lighting projects, with a construction value of $1,000,000 or greater, completed in the past five (5) years that demonstrate the bidder's ability to successfully complete the project with respect to project size, cost, use, complexity, etc. DO NOT include projects currently under construction. b. All submitted projects must be within the immediate nine Bay Area counties (Defined as Alameda, Contra Costa, Marin, Napa, San Francisco, San Mateo, Santa Clara, Solano, and Sonoma counties, California). 2. Documentation that the bidder's Project Manager and Superintendent have successfully completed a minimum of two (2) comparable projects in the past five (5) years that meet the criteria listed in 1a above. Provide resume with comparable projects listed. 3. Include the following for each project: (a) project name, (b) project address, (c) owner's name, (d) contract value at award, (e) total Change Order value, (f) completion date, (g) Owner's Representative email address and current phone number and (h) describe how project is comparable to project being bid. 4. List all ADA Accessible paths and lighting projects on all UC Campuses and National Labs (LBNL, LLNL) the bidder has completed or currently working within the last 5 years, if any.
